Why Is One Side of My Lip More Swollen Than the Other 2 Days After Lip Filler?
Day two is the day lip filler looks its worst, and for many people it is also the day it looks lopsided. If one side of your lip is puffier than the other, the answer is usually yes, this is normal, and it usually evens out over the following days.

Quick answer: One side of the lip often swells more than the other two days after filler, because swelling tends to peak between 24 and 48 hours and the two sides never react identically. Clinic guidance says it usually settles within about a week to two weeks. Increasing pain, pale or dusky skin, or a hard lump means contact the clinic the same day.
"Day two is the day my phone lights up," says Lana Zevnik, owner and specialist at Lana Skyn in Amsterdam Noord. "Day one, clients expect swelling. Day two, they expect improvement, and instead the left side is twice the size of the right. They panic. I tell them to stop judging their lips and start photographing them. Judge on day ten."

Is it normal for one side of the lip to be more swollen 2 days after filler?
Quick answer: Yes. Uneven swelling in the first days is common and usually temporary.
A UK clinic guide says swelling may increase over the first 24 to 48 hours before it goes down, and that "one lip might swell more than the other". A UK plastic surgeon's guide says uneven lips in the first week are usually caused by inflammation rather than the filler itself.
So an uneven lip on day two is a swelling picture, not yet a result.
Why does one side swell more than the other?
Quick answer: Because the injections are never identical on both sides, the tissue reacts differently, and most people start with slightly uneven lips.
Three things usually add up:
Your starting point. Very few lips are perfectly symmetrical. Filler makes an existing difference easier to see.
Your body's reaction. Hyaluronic acid draws water, and one side can simply hold more of it. Bruising on one side adds to the swelling there.
The injection itself. The amount, depth and path of the needle differ slightly side to side. Sources name technique, anatomy and how the body reacts on each side as the usual reasons.
"Everyone has a boss lip," says Zevnik. "One side that moves more, sits higher, swells more. Filler doesn't invent that. It turns the volume up on it. I would rather tell you at your consultation that your left side is the stronger one than let you discover it on day two in the mirror."
What does lip filler swelling look like day by day?
Quick answer: Swelling is usually biggest in the first one to two days, eases after about 48 hours, and most of it is gone by the end of the first week. Final shape can take up to two weeks.
Day | What many people notice | Usually normal? |
0 to 1 | Puffy, tight, sometimes one side more | Yes |
2 | Often the peak. Uneven swelling is common | Yes |
3 to 4 | Swelling easing, bruising may appear | Yes |
5 to 7 | Most swelling gone, small lumpy feel may remain | Usually |
8 to 14 | Shape settles, final result becomes visible | Expected |
After 14 days | Still clearly uneven or swollen | Time to book a review |
Sources disagree slightly on when swelling peaks, so treat this table as a guide, not a promise. Filler type, amount and your own body all change it.
What can I do while one side is more swollen?
Quick answer: Leave it alone, protect it and track it. Follow your own clinic's aftercare first.
Take one photo a day in the same daylight, same angle. Swelling changes faster than your memory.
Use cold compresses only if your clinic advises it, with a barrier between ice and skin.
Skip hard workouts, heat and alcohol in the first days, as many aftercare guides advise.
Check before taking blood-thinning medication or supplements. Guides advise doing this only after talking to your practitioner.
Do not press, squeeze or massage your lips unless your practitioner told you to.
Do not book a dissolver on day three. Wait for the swelling to go down before anyone changes anything.
"Please don't massage your lips because a video told you to," says Zevnik. "Massage is an instruction from your injector for a specific filler and a specific spot, not a general wellness habit. And never make the decision to dissolve in the first week. You are judging a swollen lip. Wait."
What do clients say about the lopsided days?
Lana hears the same sentence again and again: "I felt like I'd lost a fight with a bee on one side, and by the weekend I looked like me again, just a bit better."
"Clients love that it looks like their own lip on a good day," says Zevnik. "The lopsided days are the price of admission, not the product."
When is uneven lip swelling after filler not normal?
Quick answer: Contact the clinic the same day if pain is increasing, the skin looks pale, grey, blue, net-patterned or dusky, the lip feels cold or hard, or swelling is spreading. Call 112 for vision changes, trouble breathing or swelling of the tongue or throat.
Most uneven swelling is just swelling. The rare problem is a blocked blood vessel, called a vascular occlusion. A review in the Journal of Clinical and Aesthetic Dermatology explains that an arterial blockage usually shows up immediately with sharp pain and blanching (the skin going pale), while a blockage caused by pressure from swelling can appear later, with dull pain or no pain at all and darker skin discoloration. The same paper notes that bruises do not blanch, and that pain may be masked by local anaesthetic.
Looks like | Swelling | Bruise | Warning sign |
Colour | Normal or pink | Purple, blue, yellow-green | Pale, white, grey, blue or net-like |
Feel | Tight, warm, tender | Tender, mild | Cold, hard, pain getting worse |
Over time | Improves | Fades | Does not improve or worsens |
Contact us the same day if you notice:
Pain that increases instead of easing after the first day
Skin that turns pale, white, grey, dusky or net-patterned
A cold or firm patch on or near the lip
Blisters, scabbing or open skin
A hard lump that is getting larger or painful
Fever or pus
Swelling that is still clearly uneven after 14 days
If you cannot reach your clinic, call the huisartsenpost. For vision changes, trouble breathing or swelling of the tongue or throat, call 112.
"Send me a photo in daylight and tell me what time you had your treatment," says Zevnik. "If I can't tell from a photo, I will see you the same day. Please don't wait until Monday because it's the weekend. Dutch politeness is lovely, but a bad aftercare plan."
Who should be injecting my lips in the Netherlands?
Quick answer: Reporting by NOS has described Dutch rules as requiring filler treatments to be done by, or with, a suitably BIG-registered doctor or nurse. Ask about it before you book.
A Dutch dermatologist quoted in that article said poor technique, such as too much filler or filler in the wrong place, causes nearly all complications. The article dates from 2019, so ask any clinic about registration, and who handles complications, before you book.

Frequently asked questions
1. Is it normal for one side of my lip to be more swollen 2 days after filler? Yes. Swelling often peaks around 24 to 48 hours and it commonly affects one side more. It usually evens out over the following days. Contact your clinic if pain is increasing or the skin looks pale or dusky.
2. How long does uneven lip swelling last after filler? Most swelling settles within about a week, with the final shape visible in up to two weeks. If your lip is still clearly uneven after 14 days, book a review with your injector.
3. Should I massage my lip to even it out? No, not unless your practitioner told you to. Massaging a swollen lip yourself can cause bruising and does not tell you how the filler will settle.
4. Can I dissolve filler if my lip looks uneven after two days? It is too early to judge. Swelling hides the real shape, and guides advise waiting at least a week before deciding anything. If there is a real problem, your practitioner can discuss dissolving it.
5. How can I tell swelling from a vascular occlusion? Swelling looks pink or normal and improves. A vascular occlusion can cause increasing pain, pale, grey or net-patterned skin and a cold or firm area, sometimes with little pain. A bruise does not blanch. If unsure, send a photo and call the clinic the same day.
6. Where can I get lip filler in Amsterdam? Lana Skyn treats clients at its Amsterdam Noord clinic (Bezaanjachtplein 281) and Utrecht, beginning with a consultation.
